The Physician Orders for Life-Sustaining Treatment (POLST) is a highly effective communication tool and documentation of your wishes in the form of medical orders on a bright pink form.  

You may download and print this form. Fill it out completely and have your doctor sign it. Then take it to any print shop and have both pages printed on bright pink paper.
