For Physicians

Compassion & Choices of Oregon provides information about aid-in-dying to you, your patients, and other medical providers. Our Medical Directors are available to answer your questions. We can help coordinate the aid-in-dying process and can provide all the information you need to comply with the law (Oregon Revised Statute 127.800-127.995).

We recommend every patient who is considering aid-in-dying be referred directly to Compassion & Choices of Oregon by calling 503-525-1956 to ensure he or she has considered all options and has ongoing support through the process, including our presence at the time of ingestion, if requested.

Here is a Prescribing Physician Checklist that you may find useful.

Your patient is required to complete the Request for Medication Form.

When you are ready to prescribe, contact us for the protocol by calling 503-525-1956 or by email info@compassionoforegon.org. We provide medical protocols only to physicians and pharmacists.

We do not recommend using the one-page Prescribing (“Attending”) Physician Short Form.  If it is used, Oregon State Department of Human Services must determine compliance based on your medical record that must contain all data required on the standard two-page compliance form.

When your patient dies, you must fill out pertinent sections of the 6-page Prescribing (“Attending”) Physician Follow-up Form or call us to assist you.
