Your Advance Directive

Your advance health care directive is a set of specific instructions about what actions should be taken for your health in the event you are no longer able to make decisions due to illness or incapacity, and appoints a person to make healthcare decisions on your behalf.

Compassion & Choices of Oregon provides state-specific advance directives for Oregonians free of charge. We also provide a set of additional documents to expand and continue to define your end-of-life choice. All documents linked to this page are PDF documents.
